Digital Equity Survey
Please fill out the following form based on how your students use electronic devices to complete schoolwork at your home. You should answer the questions below based only on the conditions at your home address.
Please use your email address that we have in Infinite Campus. We will use that email address to match this information to your students. This is new information we are being asked to collect by the Minnesota Department of Education. Survey will close on 9/21/2021 at 4:00 PM.
